Most insurance companies have criteria to determine whether they feel a service is “medically necessary”. This often means that each type of service is linked with certain diagnoses for which that service has been deemed appropriate. If your diagnosis does not meet the criteria for “medically necessary”, your claim may be denied. This does not mean that the service was inappropriate or unnecessary.
A: In most cases, procedures are covered by your insurance plan, but each insurance carrier has their own list of covered benefits.
A: All HMO’s require a referral from the patients Primary Care Physician for both the office visit and any procedures or tests ordered. If your insurance plan requires a referral for your office visit, please be sure to bring one with you to your office appointment.
